Do you have patients with chronic and/or complex health care conditions?

SOCIAL WORKERS ARE NOW AVAILABLE

GP Social Work Connect is a free, short-term service available to all GP patients within the Ryde/Hornsby/Ku-ring-gai Local Government Areas (LGAs).

Primary & Community Care Services (PCCS) Social Workers are available to see patients either via home visits or at the GP’s practice. The GP Social Work Connect program is a resource for General Practice to assist with:

  • Assessing and addressing psycho-social needs and issues
  • Providing information to patients
  • Arranging referrals and linkage to appropriate services
  • Liaising with social, welfare and community providers.

Eligibility

Suitable patients should have one or more of the following:

  • Chronic and/or complex health care conditions
  • A Carer who requires support and assistance
  • Difficulty navigating and accessing support services
  • Recent hospitalisation and at risk of being without support
  • Willingness to engage with services.

To make a Referral

Complete a GP Social Worker Connect Referral Form available from

www.pccs.org.au/programs-services/gp-social-work-connect/ and fax to 1300 067 747.

All referrals will receive a confirmation via ‘secure messaging’ EDI: GPSocial.

GP Social Work Connect

GP Social Work Connect is a service for patients with chronic and/or complex health care conditions. Social Workers are now available to see patients either via home visits or at the GP's practice.

This is a free, short term service to all GP patients within the Ryde/Hunters Hill/Hornsby/Ku-ring-gai LGAs.

Primary & Community Care Services are available to assist GP patients to:

  • Assess needs;
  • Provide information;
  • Arrange referrals to appropriate services;
  • Liaise with community providers; and
  • Act as a resource to General Practice.

Suitable patients should have two (2) or more of the following:

  • Chronic and/or complex health care conditions (not mental illness)
  • Recent hospitalisations and at risk of representing without supports activated
  • Experiencing difficulty navigating and accessing support services
  • A carer who requires support and assistance
