Partners in Recovery (PiR) is seeking referrals

The Northern Sydney Partners in Recovery program (NSPIR) is seeking referrals from GPs and AHPRA registered healthcare providers.

The NSPIR program aims to better support people with severe and persistent mental illness with complex needs, their carers and families, to access services across multiple sectors in a more coordinated and integrated way. The program will promote a collaborative recovery model, strengthen partnerships and improve referral pathways. NSPIR will facilitate better coordination of, and easier access to, the key services people want and need.

To be eligible for NSPIR, an individual must:

- be living with diagnosed mental illness that is severe and has lasted, or will last for a long time.
- be between 25 and 65 years old
- live in Hornsby, Ku-Ring-Gai, Ryde or Hunters Hill LGAs
- require new supports from two or more health, social, welfare or community services (e.g. psychologist, housing, education) and not be currently receiving assistance to access these services effectively
- want to work towards recovery and consents to referral of the NSPIR program.
