Invitation to participate in timely diagnosis of dementia project

General practices are invited to submit their Expressions of Interest to participate in a scoping project to explore the timely diagnosis of dementia in Primary Care from May to July of this year.Your practice's input is vital to the projects outcomes and the information you provide will help inform strategies to support improved practice in the future. Selected participants will receive a payment of $500.

The purpose of this is to understand current practice, factors influencing current practice, and to identify what further support and development is required to support timely diagnosis and management of dementia in the future. Your practice will be asked to complete one online survey for your general practice by Friday 20 June 2014. A link to the online survey will be sent directly to you at a later time.

The survey contains 11 sections and should take approximately 30 minutes to complete. Key areas covered by the survey include demographic information, your practices approach to screening and initial assessment; use of MBS Item numbers for screening and assessment; existence and use of clinical pathways for the diagnosis of dementia; referral practices; and information and training needs. The survey also includes an option to provide examples of good practice from your general practice.

The survey is designed to be answered at the whole of general practice level (not as an individual GP). As a result there may be questions that require a level of discussion with your colleagues (other GPs and clinicians within your general practice) to reflect the whole-of-practice experience. You will be provided with a PDF copy of the survey to support discussion with your practice colleagues before completing the online survey.

If you are interested in participating, please contact Janet Smith, Aged Care Coordinator, or phone 9477 8700.
