Primary & Community Care Services (PCCS) exists to improve the health of individuals in our community. We’re doing this by making local health care easier to access – smoothing out bumps and providing stronger connections between various health care providers and the consumer.

How can PCCS help?

PCCS provides nursing, occupational therapy, social work and psychological support services in the community.

Our programs and services are designed to help people to:

• improve their health and wellbeing

• better navigate the system

• access needed supports and

• avoid unnecessary hospitalisations.

Who can we help?

We help people living with long-term health conditions and mental illness, who have complex needs.

Our strengths lie in care coordination, linking and support services for people who live in the Northern Sydney region (which includes the local government areas of Hornsby, Ku-ring-gai, Ryde and Hunters Hill).

How to access PCCS programs and services

If you or someone you know thinks they may benefit from our programs or services, feel free to contact PCCS for more information. Or, contact your GP or other health provider and ask about PCCS services that may be applicable for you.

Your Voice

There will be plenty of opportunities to get involved and help shape the health of the PCCS community - through surveys, the local media, information sessions or submitting your comments to our feedback page.

Providing your thoughts and comments means ‘your voice’ is being considered when we are developing and refining the PCCS programs and services.
