Once a week, you’ll meet with your Coach to talk about how things are going. You’ll also have the option to schedule follow-up sessions to check in on your progress and make adjustments as needed.
During your first session (which usually lasts for about an hour), your Coach will take the time to get to know you better. You’ll chat about the challenges you’re facing and choose one or two issues to focus on. From there, you’ll create a problem statement that clearly defines what you’re dealing with. Then, your Coach will work with you to create a personalised plan that includes practical steps you can take to tackle the issues at hand. Your Coach can even help link you into other supports and services you may need, especially helping address challenges around some of the social determinants of health.
If you need more support than the program can offer, your Coach will be happy to help you find a more appropriate service that meets your needs.
