TRAPEZE Support for young people with chronic conditions

Trapeze is a new state wide service to support young people ages 16-25 years old with chronic conditions, as they transition from paediatric to adult services.

Trapeze, established by The Sydney Childrens Hospitals Network (SCHN), assists young people with severe respiratory compromise and/or diabetes to develop stronger links with their GPs, specialist adult services and community teams.

The Trapeze Team will assist young people without a GP to find a practice that is sensitive to the needs of young people with life limiting or complex health needs. They will also provide the GP with the relevant information to assist them in developing a GP Management Plan and Team Care Arrangements.
