NSML supporting local practices to participate in the APCC program

NSML is pleased to announce that we are currently supporting three local GP practices to participate in the Australian Primary Care Collaborative (APCC Wave 10) Program. The Cardiovascular Disease topic element focuses on improving the management ofpatients with diagnosed Coronary Heart Disease through secondary prevention.

The aims of the CVD and CKD wave are:
  • A 10% reduction in cardiac events (ACS, AMI or revascularisation procedures) over 12 months
  • To assess risk factor status (through the use of Absolute Risk Assessment tool) in 20 per cent of those in whom it is recommended and reduce the number of risk factors that are not at target by 20 per cent
  • Increase the screening for CKD to 90 per cent of the practice population who are at high risk*

*high risk includes: Hypertension, CVD, Diabetes, Obesity, Smokers, 60 years (Aboriginal Torres Strait Islanders30 years)
