Introducing NSML's Primary Health Care Integration Unit

Northern Sydney Medicare Local would like to introduce you to our newly established Primary Health Care Integration Unit. The Unit aims to improve the interface between hospital and primary care based services in the Northern Sydney ML region (Hornsby, Ku-ring-gai, Ryde & Hunters Hill) and will be involved in the development and implementation of projects which aim to improve continuity of care, through primary care to hospitals and back, improve clinical handover and facilitate delivery of care in the most appropriate setting, preventing the need for acute care where appropriate.

The Unit is comprised of:

  • Cynthia Stanton, Director Primary Care & Integration cstanton@nsml.com.au. Cynthia has worked for NSML since its establishment and previously for GPNN, in various roles over the previous 10 years, including the former GP Collaboration Unit and worked with NSLHD to set up the Northern Sydney HealthOne service. She will oversee the Units development and implementation.
  • Dr Alexandra Fletcher, GP Liaison Officer afletcher@nsml.com.au Alexandra has had several years of experience as a GP both within the Northern Sydney region and outside of it. She will be working closely with the NSLHD and local primary care providers to improve the current primary/acute healthcare interface and act as a key clinical point of contact.
  • Kim Kindler kkindler@nsml.com.au and Sandra Collins scollins@nsml.com.au Primary Health Care Integration Unit Coordinators. Kim and Sandra both have a background in pharmacy and have been delivering the NPS educational program to local GPs for over ten years. They will be working alongside Alexandra to help develop the PHCIU and to coordinate the day to day operation. They are also key points of contact for GPs.

The PHCIU looks forward to working with GPs to improve the patient journey through hospital and back again, and welcomes communication from GPs about any issues they might be experiencing.
