GP survey to help improve Diabetes and Chronic Kidney Disease Services

This large multi-centre research project, called the Diabetes Renal Project (DRP), is being conducted by Associate Professor Sophia Zoungas, Monash University, in partnership with Monash Health, Alfred Health, Royal North Shore Hospital, Concord Repatriation General Hospital, The George Institute for Global Health, Diabetes Australia, and Kidney Health Australia.

This project aims to address the critical gap between recommended practices, treatment targets and the care currently received by people with diabetes and chronic kidney disease. As a General Practitioner, your opinion is valued as to how health services can improve to meet the needs of these patients.

Monash University - in partnership with Monash Health are very interested in obtaining your feedback on your experiences with the above mentioned tertiary public hospitals.

Results from this project will assist in developing and evaluating a new model of care to improve patients clinical outcomes and quality of life.
