Acute Post-Acute Care (APAC) program referral pathway

The APAC service is a Hospital in the home service that enhances patient care options and open new avenues for which General Practice can access patient care.

The service works closely with GPs to treat patients in their home (including nursing homes) as an alternative safe option to hospitalization.

GPs may refer their patients to the service by phoning1300 732 503 between 7am 10.30pm seven days per week.

Some of the benefits of the program include:

  • Reduction in, the length of hospital stays and, the adverse events associated with hospitalisation
  • Collaboration and partnerships between GPs and APAC clinicians
  • Multidisciplinary team approach to patient care
  • Patients are treated in their home environment

Many conditions may be appropriate for APAC referral and GPs are welcome to discuss the suitability of the service on a case by case basis. Currently, the most prominent conditions referred are Dehydration; Community Acquired Pneumonia; Chronic Obstructive Pulmonary Disease; Cellulitis; DVT an Urinary Tract Infections.
