GP Social Prescribing for Injured Workers

Pilot Program

What is GP Social Prescribing?

PCCS's Plus Social GP Social Prescribing program is for injured workers who may be isolated, losing confidence, or losing their direction. The program addresses the wider social, welfare and lifestyle aspects of their wellbeing.
 

PCCS's GP Social Prescribing program includes:

  • Access to non-clinical services (e.g. financial counselling, community services)
  • Opportunities to meet other people and build support networks
  • Free group activities such as exercise, art or mindfulness

 

Who can be Referred?

Injured Workers:

  • who have a Certificate of Capacity from an Australian Workers Compensation Scheme and have been unable to return to work or returned on reduced hours
  • who have psychosocial needs that are impacting their quality of life
  • who would benefit from linking to services and group-based activities.

 

Why Participate?

  • Link workers will support your patients to meet their psychosocial/non-medical needs with connections to services provided by NGOs, social and welfare services
  • Patients experience increased confidence and reduced isolation through social participation
  • International studies indicate improvements in general health, well-being and quality of life.

 

Referral Process
 
Assess your patient for eligibility and refer your patient to the program using the GP Social Prescribing Referral Form.
